Fees & Insurance

I provide the client with a superbill to submit to their medical insurance for reimbursement. A superbill is a form with the information your health insurance needs to process the payment and reimburse you. A superbill has the client's name and birthdate, my business information and license information, the ICD codes and the CPT codes required, and the amount the client has paid.

I suggest you call your insurance provider before our first session to ensure that they will honor this superbill plan. Most do.
